Your Pre-Employment Physical

Your recruitment team will contact you to schedule a pre-employment physical.  We have several options available to meet your schedule.  Your physical may include:

• General pre-employment physical
• Eye test
• Review and administration of immunizations including a flu shot
• TB skin test
• Fit test

Documentation to Bring

Please print the following forms and bring them with you to your physical appointment:

• Consent for Drug and Alcohol Screen (PDF)
• Consent to Physical Examination (PDF)
• Health History (PDF)
• Vaccine Exemption - Medical (PDF)
• Vaccine Exemption - Religious (PDF)

*In addition you may want to bring a shot record, proof of flu shot/tb skin test if within the last year, and a list of any medications that you are currently taking.
