This is a transformational outreach program that seeks to extend the resources of Methodist Le Bonheur Healthcare (MLH) by collaborating with local partners to create a healthier community.
The basic model is for MLH to enter into a covenant relationship with area congregations, pastors, and members as a commitment to deliver healthcare education, awareness, messaging, and resources in as many ways as possible. MLH deploys the CHN and its full-time navigators to connect to churches; churches and their volunteer liaisons connect to pastors and church members; and ultimately, pastors and church members connect to wider neighborhoods, including otherwise underserved areas. In this way, the CHN builds a network of trust, communication, and influence to move the needle on community health needs.
Over the lifetime of the CHN, about 700 different churches and community organizations have formed covenant agreements to partner with MLH in this work. The number of actively engaged partners fluctuates with needs, resources, and priorities in the community, but the reach of the network is considerable and sustainable. The CHN works through many different kinds of programs that focus on support, interventions, and resources.
