Our Assessment
A community health needs assessment (CHNA) is a critical tool that aids various agencies in identifying the most pressing health issues of a community while providing a contextual foundation of how an agency can raise awareness and implement an action plan to combat certain community health problems.
MLH views the undertaking of the CHNA as an opportunity that affords our system the ability to listen better and learn of the community’s health needs, and enhance a strategy that improves and protects the health and wellbeing of patients and their families.
