Social Determinants of Health

Seeking to address the holistic health needs of our patients and community, Methodist Le Bonheur Healthcare is embarking on a journey to create a comprehensive strategy for addressing the social determinants of health.

Whether our patients and community members, struggle with access to nutritious food, lack transportation, face financial barriers including poverty and unemployment, have low health literacy, or are unable to access safe places for physical activity and healthy living, we are committed to identifying meaningful ways to remove these barriers and help our community members promote and maintain their health and well-being.

Your support will help ensure that every patient we serve has access to the health care that they need, are able to maintain their health, and can live with dignity and hope.

As programs and resources are developing, Methodist Healthcare Foundation looks to partners like you to bring this vision and work to life through your donations to “Paula’s Fund for the Underserved”, a fund named in named in honor of Paula Jacobson who served as President of the Methodist Healthcare Foundation for 18 years.

Donations to this Fund will be used to address the many social barriers faced by our most at-risk and underserved patients, such as: financial assistance to patients for medical costs and prescription needs, transportation to and from health care appointments, basic hygiene supplies, and food.
