Accessing Medical Records in MyChart
MyChart allows you to view, download and share your medical records.
- In the Menu, go to Sharing and click on “Sharing Hub.”
- Select “Download health and visit summary.”
- Choose a visit on the Single Visit tab or select multiple visits by using the Date Range tab or All Visits tab. Then take one of the following actions:
- Click “View” to view a copy of the visit summary.
- Click “Download” to save a copy of the visit summary for your records.
- If you would like to send a copy of your visit summary to a provider who works outside of your clinic, click “Send.”
You can also download formal copies of your health records to give to other organizations, such as your workplace or insurance company. Under Sharing, select “Sharing Hub” and click on “Request formal copy of health record.”

Requesting Copies of Medical Records
You can request copies of your medical records, depending on the type of information you need. To submit a request for a copy of your medical records, click here.
To request a copy of your medical records for your healthcare provider or third party, complete a Medical Authorization Form, and provide a copy of your photo ID.
Submit the form by:
Mail: MLHC - Medical Records, PO Box 40628 Memphis, TN 38174
Electronically: To submit a request for a copy of your medical records, click here.
Walk-In: You may leave an authorization/request for your medical records at any MLH facility with the Registration desk associate, and it will be processed within ten business days.
Please ensure that all fields are completed in order to ensure timely completion of your request. A copy of your valid photo ID must be included with your request.
If the patient is unable to sign, an authorized personal representative may sign this form. Written proof of authority should accompany the request in order to verify appropriate health information.
Records of Deceased Patients: To obtain a copy of a deceased patient’s record, you must complete, date and sign a Medical Authorization Form. The authorization form should be completed and signed by:
- Next of Kin
- Executor of Estate or Court Order Administrator
- If there is no Executor of Estate or Court Order Administrator, a notarized Affidavit of Next of Kin form must be completed.
A death certificate and additional proof of authority may be requested as appropriate.
