We accept cash, checks, MasterCard, Visa and Discover. You can pay:

If you would like to request a copy of your medical records from Methodist Le Bonheur Healthcare (MLHC) at any time, complete the medical authorization form and mail to the P.O. Box below.

Please insure that all fields are completed in order to ensure timely completion of your request. A copy of your valid photo ID must be included with your request.
If the patient is unable to sign, the requestor must provide DURABLE POWER OF ATTORNEY documentation along with a completed authorization form.
