Hospital pre-registration can make your check-in at the hospital easier. Silver Stork pre-registration packets are available online, at your doctor's office or by calling the hospital where you will deliver.

Or, you can download the Silver Stork pre-registration form (PDF). Fill the forms out completely and mail them to your delivery hospital when you are about five months pregnant. 

When you arrive at the hospital, give your name and your registration information will be available. You will still need to answer a few health history questions, but this pre-registration will decrease the amount of paperwork you have to fill out.
