Please bring the following items with you to the surgery center:

• Insurance cards and driver's license or photo identification. You may have provided this to your physician's office, but the surgery center will also need to make copies of this information.
• Parent or legal guardian of children younger than 18 years old.
• A list of all regular medications, both prescription and non-prescription medications.
• Advanced directive, if you have one. This is a document that outlines the medical treatment you would want or identifies the person you want to make health care decisions for you if you are unable to do so.
