You will be asked to complete a questionnaire regarding your health history and physical state at your physician's office.  You may also be asked to complete a medication form. This information will be sent to the anesthesiologist and staff at our surgery center. The anesthesia team will review the information and may contact you if they have questions or need to give you specific instructions. Otherwise, they will meet with you the day of your scheduled surgery.
