Hospital Paperwork

When you enter the hospital to deliver your baby, it's best to have all of your paperwork filled out ahead of time. This will make it a little less stressful. There are just a few forms that you cannot fill out during the pre-registration process, and these will be given to you when you arrive.


Birth Certificate 
  Before you leave the hospital, a maternity center staff member will help you complete the form for your baby's birth certificate.
Permission Forms 
  Know what you're signing before you sign. When you are admitted to the hospital, your nurse will give you several permission forms to sign. It is important that you read and understand the information on each form.
